Drawing on Danish national registries, this nationwide study included individuals diagnosed with ulcerative colitis (UC) or Crohn's disease (CD) who were biologically naive when commencing treatment with infliximab, adalimumab, vedolizumab, golimumab, or ustekinumab between 2015 and 2020. Employing Cox regression, we determined the hazard ratios associated with discontinuing the first treatment or switching to an alternative biological regimen.
Analyzing data from 2995 UC and 3028 CD patients, infliximab was the initial biologic treatment in 89% of UC patients and 85% of CD patients. Adalimumab (6% UC, 12% CD), vedolizumab (3% UC, 2% CD), and golimumab (1% UC) followed for UC, and adalimumab (12% CD), vedolizumab (2% CD), and ustekinumab (0.4% CD) for CD. When comparing adalimumab as the first treatment series to infliximab, a higher treatment discontinuation risk (excluding switching) was observed in UC patients (hazard ratio 202 [95% CI 157-260]) and CD patients (hazard ratio 185 [95% CI 152-224]). The study of vedolizumab versus infliximab revealed a lower risk of treatment discontinuation for ulcerative colitis (UC) patients (051 [029-089]), and a non-significant decrease in discontinuation rates for Crohn's disease (CD) patients (058 [032-103]). Programs supporting the psychosocial well-being of patients receiving craniofacial care, based on solid evidence, are unfortunately few and far between. The present investigation evaluated the implementation potential and patient satisfaction associated with the Promoting Resilience in Stress Management-Parent (PRISM-P) intervention among parents of children born with craniofacial differences, and explored the hindering and enabling factors influencing caregiver resilience to inform future program modifications.
Participants in a single-arm cohort study underwent a baseline demographic questionnaire, the PRISM-P program, and a final exit interview.
Children under twelve years of age with craniofacial conditions had English-speaking legal guardians who were eligible.
Four modules (stress management, goal setting, cognitive restructuring, meaning-making) constituted the PRISM-P program, delivered in a sequence of two one-on-one phone or videoconference sessions, occurring one to two weeks apart.
Program completion exceeding 70% amongst enrolled participants signified feasibility; the criterion for acceptability was a recommendation of PRISM-P by over 70% of participants. After being approached, twelve (60%) of the twenty caregivers agreed to join.
